General Responsibilities: 

  • Performs physical and psychological nursing assessments on residents when necessary.

  • Reports all unusual circumstances that may involve the safety and well-being of the residents and staff directly to the Assistant Director of Nursing or Director of Nursing.

  • Demonstrates leadership ability and management skills through fair decision-making, dependability, and reasonable expectations.

  • Acts as house supervisor at prescribed intervals.

  • Coordinates daily operations on the unit to ensure quality clinical care and patient satisfaction.

  • Maintains quality care by reviewing the daily census, staffing, and pending admissions/discharges, and adjusts assignments accordingly.

  • Consults with the nursing staff concerning the implementation of resident’s plans of care.

  • Reviews doctor’s orders and receives all new orders to coordinate appropriate changes or services.

  • Attends and/or leads meetings with nursing staff, nursing administration, and/or physicians.

  • Evaluates subordinate professional and sub-professional nursing personnel.

  • Identifies and resolves obstacles associated with unit processes and procedures.

  • Coordinates room changes with nursing staff and admissions.

  • Collaborates in patient review with MDS nurse.

  • Discusses patient needs with other departments, such as Restorative Therapy and Life Enrichment, and coordinates multiple services needed for resident care.

  • Identifies and refers residents needs to contract therapies such as OT, PT and speech therapy.

  • Provides professional nursing care by determining the kind and amount of nursing care required.

  • Oversees the admissions and discharge processes to ensure smooth transition for all residents.

  • Makes walking rounds between shifts and whatever necessary to supervise and monitor performance in terms of resident care, staff regulations and efficiency of service.

  • Contacts families of residents regarding issues that may arise such as medical or behavioral problems.

  • Informs physicians of any important medical changes and conditions.

  • Disciplines or effectively recommends such action for assigned staff.

  • Functions as nursing representative on the unit during any licensing or accreditation survey.

  • Keeps current with job related professional developments, regulations and/or operational procedures (IDPH, Medicare, CMS, etc.) impacting unit operations.

  • Participates in facility-wide quality improvement initiatives and projects.

  • Plans and coordinates resident’s plan of care with other members of the interdisciplinary team.

  • Discusses resident’s condition with physicians, nursing staff and family members.

  • Participates in care plan conferences and other interdisciplinary meetings as needed.

  • Reviews resident charts for contradictions in information and researches any contradictions to determine what is valid.

  • Identifies self as primary contact to families and clients.

  • Maintains professional status with physicians.

  • Responds to all emergency situations within the facility.

  • Serves as a role model for nursing employees.

  • Cooperates with all inspections of nursing care by an outside agency.

  • Demonstrates consistent ability to identify problems and report in an objective manner to take appropriate corrective action.

  • Observes all facility safety policies and procedures and assumes leadership role.

  • Directs restorative unit programs.

  • Responsive to nursing staff during on-call rotation.  This may include working in a direct care capacity (Nurse or CNA) as the need arises to ensure resident and staffing needs are being met

  • Performs other duties as assigned.
